PRE ANESTHESIA SCREEN CLINIC
A few days before your surgery, the anesthesia department will call you and let you know what type of Pre Anesthesia Screening appointment you will require. Whether it is a phone call or a visit to the clinic, you will speak and/or meet with a nurse, an anesthesiologist, a pharmacist or other practitioners depending on your needs. Please know that the appointment can last up to four hours,
Make sure you have the following information ready:

Information regarding your Medical History and Current medications

Completed Pre-Anesthetic Questionnaire given to you by your Surgeon

Any forms given to you by your Doctor including History and Physical
-
Ontario Health Insurance Card, additional photo Id for those patients holding a red and white Health Card
-
Extended Health Coverage information
-
Bring all medications that you are currently taking in the original containers, inhalers and over the counter drugs
-
Bring an interpreter if English is not your first language

The Hospital facilities are SCENT FREE, please don't wear perfume or fragrance of any kind.

If your surgery is at Brampton Civic Hospital, your Pre Anesthesia appointment will be at the
Brampton Civic (905)- 494-6591.

If your surgery is at Etobicoke General Hospital, your Pre Anesthesia appointment will be at
Etobicoke General Hospital (416) 747-3490.
​
If you have to cancel your Pre Anesthesia appointment please call the number above.

Please arrive 20 minus prior to your appointment to register.
