Our current wait list is approximately 9 months, depending on the circumstances of the patient.   Please refer pediatric patients directly to SickKids.

FOR SHOULDER PATIENTS:

Please forward current Hockmann View X-Ray (Images and report) and an MRI Scan (if available) with the referral.

FOR ELBOW PATIENTS:

Please forward current Elbow x-rays should include AP, Lateral and Oblique views.

IT IS IMPERATIVE THAT ALL PATIENTS HAVE A RECENT X-RAY AND MRI SCAN ON A CD PRIOR TO COMING TO OUR OFFICE.

It often takes up to 3 months for an appointment to be scheduled.  Once the appointment has been scheduled, we will contact you by a fax referral letter stating the time and date of the appointment along with the pertinent information required for the patient to bring to the office appointment.

Please do not resend referral but instead, call the office to follow-up on the status of your request.
